Fees and Policies
New clients are required to complete and submit an intake form and informed consent for treatment; these documents and federal HIPAA guidelines will be emailed to a new client prior to the first session. Documents requiring completion and or signatures are due upon the start of that session.
Confidentiality in therapy is key; CA state law requires licensed mental health professionals to break confidentiality if a client is a danger to themselves, others, or have or are intending to harm a child, elderly person, or a dependent adult.
As a result of the federal No Surprises Act, all health care providers and health care facilities licensed, certified or approved by the state must provide “good faith estimates” of expected charges for services and items offered to uninsured and self-pay patients.
All clients will receive a copy of the Good Faith Estimate notification and an individual estimate.
Myers Wellness is a fee for service entity; payment is due the day services are provided.
Clients in need of a monthly invoice for reimbursement purposes will be accommodated.
Appointment reminders via text are a courtesy; please do not expect a reminder. Late cancellations (notification to Jan Myers less than 24hours prior to scheduled appointment) or a no show are $50, due the date of said appointment.
Jan Myers is unavailable generally between 9p and 8a. If an individual is in crisis, call 9-8-8 (effective 7/17/22). Other immediate options include calling 9-1-1, go to the closest emergency room, or go to your county behavioral health crisis unit.
